    Introduction: Subarachnoid hemorrhage is a common neurological disorder that can have added complications such as systemic inflammatory response syndrome has been associated with symptomatic cerebral vasospasm, however the behavior of it and how it relates to other complications is unknown. Objective: To assess by neutrophil / lymphocyte ratio behavior that keeps the systemic inflammatory response in patients with aneurysmal subarachnoid hemorrhage origin. Methodology: 17 patients were evaluated for 15 days from admission to the National Institute of Neurology and Neurosurgery "Manuel Velasco Suarez" in order to describe the behavior that keeps the systemic inflammatory response throughout its evolution and the relationship with the appearance of associated complications such as vasospasm, hydrocephalus, and electrolyte abnormalities. Results: 17 patients of whom 76% had electrolyte disturbances were analyzed; 47% vasospasm and hydrocephalus in 35%. In relation to vasospasm, 30% culminated in areas of infarction and increased value to 10 of the neutrophil / lymphocyte ratio to income directly correlated with the occurrence of vasospasm (p <0.001). Conclusion: The behavior of the systemic inflammatory response in patients with aneurysmal subarachnoid hemorrhage origin is different in those who develop vasospasm, for which there is a direct relationship between (greater than 10) high values of the neutrophil / lymphocyte ratio with the appearance of symptomatic cerebral vasospasm and hydrocephalus.

    Introducción: tradicionalmente, la comprobación de la correcta ubicación del catéter venoso central se realizaa través de radiografía de tórax, pero existen otras alternativas. Objetivo: describir la técnica basada en el registro electrocardiográfico intracavitario, sustituyendo a la fluroscopía y a la radiografía de tórax como método preferente para localizar la punta del catéter.La localización de la punta del catéter venoso central con el método del electrocardiograma intracavitario (ECG-IC) proporciona información en tiempo real sobre la colocación del extremo proximaldel catéter usando la actividad eléctrica del corazón.2-4 Metodología: Se realizaron búsquedas en buscadores como PubMed, ScienceDirect con los términos “cateter venoso central”, “electrocardiografia y anestesia”, “arritmias y colocación de cateter venoso central”. Resultados: El método de ECG-IC se considera el estándar de oro, por la seguridad, bajo costo y disponibilidad en el transoperatorio. Conclusiones: Aumenta la seguridad global del procedimiento y disminuye el riesgo de arritmias iatrogénicas.2 Esto evitaría la exposición a radiación innecesaria de las radiografías y potencialmente permitir ahorros de tiempo, que pueden ser especialmente importante en el cuidado de pacientes en estado crítico.10

    The purpose of this research is to increase the strength and capability of the Integrated Fleet Weapon System by procuring the main weapon system (defense equipment) with the latest technology. Meanwhile, increasing the professionalism of soldiers by carrying out education and training. The Education of the Naval Command and Staff School is directed at educating and strengthening the middle officers of the Indonesian Navy and the Indonesian National Armed Forces. The analysis method is carried out through explanatory research, using PLS/SEM applications. Concepts and problems under study look at causality, then explain the variables causing the problem under study. The research sample was 130 leaders and staff of the Naval Command and Staff School. The results of this study transactional leadership, strategy planning affect organizational performance. Knowledge management has an effect on organizational performance. Likewise transactional leadership, strategy planning has an effect on organizational performance which is mediated by knowledge management at the Naval Staff and Command School. Theoretical implications with the existence of transactional leadership, strategy planning through increasing its dimensions will be able to improve the organizational performance of the Naval Staff School and Command so that it can increase the trust and loyalty of related parties. The managerial implication that needs to be implemented is that leadership, planning and strategy are very influential on organizational performance, so increasing transactional and strategic leadership through improving every dimension of policy and strategy must be a priority to be able to maintain the organizational performance of the Naval Staff and Command School.

    El espacio supratentorial contiene la porción cerebral localizada por encima de la cubierta de duramadre conocida como tienda del cerebelo.Existen diferentes mecanismos de compensación que intentan mantener la PIC en rangos normales ante el crecimiento de un tumor supratentorial.Al agotarse los mecanismos de compensación, el tumor ocasiona cambios en la PIC, incluso con el más mínimo crecimiento. El desplazamiento del tejido cerebral es la base de los síndromes de herniación representando el estadio final de los procesos de compensación. En todo paciente neurológico resulta de suma importancia conocer el estado de alerta previo al procedimiento, con el objetivo de determinar la cantidad y tipo de fármacos necesarios durante el evento quirúrgico. La valoración preanestesica general no se debe dejar a un lado, dado que conocer la funcionalidad de los aparatos y sistemas nos brinda seguridad para el sistema nervioso, sobre todo si recordamos que para tener adecuada perfusión y oxigenación cerebral se necesita correcta función cardiovascular y respiratoria. Además, ciertas patologías intracraneales alteran la función cardiovascular y por otro lado, en el caso de metástasis cerebral alrededor del 40% tienen origen pulmonar, repercutiendo en el funcionamiento respiratorio. El inicio de todo procedimiento anestésico puede estar asociado a cambios hemodinámicos importantes que repercuten en el pronóstico del paciente. Por lo tanto, los principales factores que debemos considerar son de tipo ventilatorio (evitar hipoxemia, hipercapnia e hipocapnia extrema), control simpático y de la presión arterial, esto se logra con adecuado plano anestésico y antinocicepción evitando la activación del sistema nervioso central, además de permitir el drenaje venoso cerebral, mediante la correcta posición de la cabeza. El objetivo de los fármacos utilizados en el mantenimiento de la anestesia es conservar un ambiente intracraneal optimo, para conseguirlo es necesario evitar la activación del sistema nervioso central con adecuado nivel de hipnosis y antinocicepción, además de cuidar el acoplamiento entre el aporte y la demanda de los sustratos cerebrales, preservando el flujo sanguíneo y perfusión cerebral. Ha existido gran controversia en torno al uso de anestésicos intravenosos y halogenados para cirugía neurológica y hasta la fecha no existe un ensayo prospectivo que compare dichas técnicas.

    Many types of the cutaneous hybrid cysts are described in literature since they were first described in nineteen sixties. These hybrid cysts are developing from Folliculo-sebaceous apocrine units in the skin. These can be single or multiple. When such type of cyst is detected, these patients are frequently examined for any syndromic association like Lowe syndrome, Gardner syndrome and Gorlin Goltz syndrome. However, literature shows that many of the cases are non-syndromic. There can differentiation towards infundibulum, isthmic region or lower parts like follicular matrix, inner root sheath and hair. Exact pathogenesis involved in the formation of these cysts is largely unknown. Follicular germinative cells are hypothesized to form these types of cysts which are capable of divergent differentiation. There are occasional case reports from India. Hereby we report rare case of cutaneous hybrid cyst in middle aged male with right forearm swelling for 8 months. On histopathological examination lesion, it showed epidermal inclusion cyst and Pilomatricoma. The patient was asymptomatic apart from the swelling. He had no gastro-intestinal problems and his family history was negative for the colorectal carcinoma.

    Los meningiomas son los tumores primarios más frecuentes del sistema nervioso central, generalmente se asocian a buen pronóstico, puede haber variables agresivas y con un comportamiento clínico desfavorable, con menos frecuencia. La resección microquirúrgica y el grado de la misma son el pilar del tratamiento y el predictor de progresión. Se presenta el caso de una mujer de 53 años con un meningioma del ala menor del esfenoides izquierda, que se presentó como una imagen quística con un nódulo en la pared, lo cual es un hallazgo infrecuente en estas lesiones; se sometió a una craneotomía pterional y resección microquirúgica Simpson I de la lesión, el estudio histopatológico reporto un meningioma atípico grado II. La radioterapia postoperatoria temprana (RT) en meningiomas atípicos sigue siendo un tema controvertido, ya que algunos estudios señalan que no hay evidencia que sugiera que la RT disminuya la tasa de recurrencia en pacientes con meningiomas atípicos.

    Background: Multiple physiological changes happen in the body during pregnancy resulting in musculoskeletal problems. Separation of the abdominal muscles which is called diastasis of rectus abdominis (DRA) is one of the common occurrences during pregnancy and after child birth. Growing uterus increases the abdominal pressure which may lead to the rectus abdominis muscles separation along linea alba. Purpose: The purpose of this study was to determine the impact of the delivery mode either normal delivery or cesarean section on abdominal rectus diastasis. Materials and methods: 284 pregnant women were enrolled in this study, ranged in age from 25 – 35 years with body mass index (BMI) between 25 – 29.9 kg/m2 and they were assigned randomly into two groups of equal number (A and B). Group (A) consisted of 142 pregnant women who delivered after normal delivery while group (B) consisted of 142 pregnant women who delivered after cesarean. Both groups were assessed regarding the strength of abdominal rectus muscles in terms of diastasis. The participated women in this study were assessed by Waist circumferences, Digital caliper, and Oswestry disability index (ODI). They were evaluated after 6 weeks postnatal and again after another 6 weeks of the 1st assessment i.e. 12 weeks from the delivery. Results: The obtained results revealed significant differences of all measured variables in the two groups. Statistically significant difference was also found of all measured variables when compared between the two groups .There Is Significant effect on Rectus Abdominis Muscle. In Cesarean Group more than normal delivery group. Conclusion: The impact of the cesarean section was more significant than normal delivery on abdominal rectus diastasis.

    Background: Iron deficiency is a common comorbidity in patients with heart failure. Iron deficiency impairs exercise capacity, reduces the quality of life, and increases hospitalization rate and mortality risk regardless of anaemia. A high prevalence of iron deficiency has been found in patients with HF with reduced ejection fraction. Our study aimed to assess the effects of intravenous iron therapy on clinical condition, left ventricular function and quality of life in patients who suffered of chronic heart failure and concomitant iron deficiency. Material and Methods: The present study compressed 62 patients (29 males, 33 females) with advanced heart failure. The study group was divided into three small groups: one that was treated with intravenous iron, one with oral iron, and the third without treatment. The duration of treatment was 24 weeks. In all, general physical, clinical, laboratory, and echocardiographic examinations were performed, before and after treatment, and compared between them, for each group. At the beginning and at the end of the study all patients underwent the Kansas City Cardiomyopathy Questionnaire. Results: The NYHA class showed a significant improvement after intravenous iron. The prevalence of heart failure-related edema was also significantly reduced from 67% before treatment to 47% after that (p = 0.036). The rate of hospitalization in the intravenous iron group was considerably reduced from 47% to 17% (P < 0.001). The treatment of iron deficiency in this group of subjects got a significant improvement in KCCQ total score. Conclusions: Iron deficiency is associated with increased morbidity and mortality in heart failure patients. Intravenously correcting iron deficiency has emerged as a promising treatment in heart failure, improving symptoms, quality of life, exercise capacity, and reducing hospitalizations.

    Ameloblastoma is a benign odontogenic neoplasm that frequently affects the mandible. The term ameloblastoma includes several clinical, radiological, and histological types. In many cases, it poses significant diagnostic and therapeutic challenges. Ameloblastoma is a benign tumor that invades locally with the most common site being the third molar region mandibular jaw. Most commonly ameloblastoma occurs in the mandibular third molar region up to 66%, followed by 11% in the mandibular premolar region, 10% in the mandibular anterior region, 6% in the maxillary anterior, and posterior region, and only 1% in the maxillary premolar region. Here we present a rare case of Unicystic ameloblastoma occurring in the anterior mandibular region crossing the midline in a 17-year-old-male.

    Background: Intrauterine Contraceptive Device is a very reliable long-acting reversible contraceptive that can be used for many years. With Nigeria's relatively low contraceptive prevalence rate, the most populous country in Africa, it is important to explore factors associated with discontinuing intrauterine devices in its users. Objective: This study determined factors associated with discontinuation of intrauterine contraceptives in reproductive aged women. Methods: This was a descriptive retrospective cross-sectional study in which 687 reproductive-aged women, who hadintrauterine contraceptive devices (IUCD), at Lagos State University Teaching Hospital Family Planning clinic, over a 5-year period, were assessed for possible discontinuation and associated factors. Data were collected with a structured proforma designed for the study and analysed with relevant descriptive and inferential statistics at p<0.05. Results: The IUCD was the most selected contraceptive, by 90% of women, in the study clinic with a discontinuation rate of 11%. Women aged 31 to 40years accounted for about 45% of intrauterine contraceptive users were the most common group of users followed by women aged 41-50years. The average number of children by these women was 3. No nulliparous woman opted for IUCD during the study period. The commonest reason for discontinuation was the desire to get pregnant (19.7%) and menorrhagia and missing strings were other common causes. The women’s age and marital status significantly influenced IUCD discontinuation. Conclusion: Relatively older reproductive-aged women more readily opted for IUCD. Single and low parity women were more likely to discontinue its use for the desire to conceive.

    Innovation has always been fueled by thinkers. William Morton revolutionized dentistry and surgery by utilizing ether as an anesthetic although he was not a doctor. The goal of anesthesia in the beginning was to prevent pain during surgical procedures; without pain, patients would not move, and surgery could be performed successfully. Now a days, the goal of anesthesia is to provide a well-rounded experience that satisfies patient’s and surgeon’s expectations. In ophthalmic anesthesia, this is especially important as patients are sedated in a particular manner according to the surgery type, the step of the surgery and the surgeon’s needs. The anesthetic experience provided in ophthalmic care is directly related to the communication and rapport the anesthesiologist establishes with both patients and surgeons. Anesthesiologists increase value in ophthalmic care by acknowledging the expectations of all those involved and implementing humanizing little details may directly impact the success of a surgical procedure.
