    ABSTRACT:

    This study investigated the correlation between teacher motivational competence and perceived outcome of Computer Studies instruction in Secondary Schools in Rivers State. A correlational research design was adopted. The population for this study comprised 379 Computer Studies teachers and 222750 students of the government owned senior secondary schools in Rivers State. The purposive sampling techniques was adopted in this study in obtaining 379 Computer Studies teachers and 18950 students, making a total sample of 19329 respondents. The instrument used for data collection was a structured questionnaire. The reliability coefficient of the instrument was determined using Cronbach Alpha method with values 0.66 and 0.79 obtained. Two research questions and one research hypotheses guided this research. Mean (x ̅), Standard Deviation (SD) and Pearson Product Moment Correlation (PPMC) was used for answering research questions while simple regression analysis and t-statistics was used to test the hypotheses of no significant relationship at 0.5 level of significance. The findings of the study revealed that the respondents agreed that teacher’s motivational competence have a notable relationship with outcome of Computer Studies instructions in Secondary Schools in Rivers State. The result of the hypotheses also indicated that there is a significant relationship between teacher motivational competence and outcome of Computer Studies instruction in Secondary Schools in Rivers State.

    In order to determine the epidemiological profile of neonatal infection in Kindu, this retrospective study was conducted on 116 cases of neonatal infection recruited in neonatology from the general reference hospital of Kindu. The clinical picture was marked by the frequency of severe forms. The bacteriological assessment allowed the confirmation of neonatal infection in 92 cases and showed the predominance of gram-negative bacilli, streptococcus B and the absence of listeria monocytogenes.Neonatal infection remains a worrying pathology in terms of its frequency and severity. Its frequency is estimated at 10 to 15% of live births.The incidence of maternal-fetal infection is around 0.6% to 1% at birth. Its severity is linked to the immuno-incompetence of the newborn and to the risk of mortality which is around 10 to 30% depending on the series.Its diagnosis is of variable difficulty highlighting the importance of the paraclinical assessment, in particular the bacteriological samples. Treatment is based on antibiotic therapy adapted to the sensitivity of the responsible germs, based on 3rd generation cephalosporin, amino penicellin and aminoglycoside associated with symptomatic treatment.

    ABSTRACT:

    This article questions personhood and normative desire in albinism in African literature in the fictional work of Tara Sullivan’s Golden Boy. It draws attention on the representation of disability in albinism and analyse how this shape identity, personhood, and normalcy in characters with albinism. Using Mary Douglas’s concept of dirt, the paper argues that the disability associated with albinism body forms a central plot of narrative resulting into negative body imagery which affects the identity and personhood of the persons with albinism. The article agrees with Frederick J. White notion of personhood as a relational construct in which personhood is a conditional state of value defined by the society. In this article I argue that the accidental quality attached to albinism does not render characters with albinism less human as they forge the identity in a hostile environment.

    ABSTRACT:

    Background: With the advancements of education sector around the world, this study aims to compile the practices of public high school teachers teaching Grade 9 Biology and determine which of these said practices are the best. Objectives: The objective of this research is to know the practices of science teachers in teaching grade 9 biology and to determine which of these practices are considered to be the best. Methods: This research utilized a qualitative-quantitative method using a structured interview form administered through Google Forms. This form was divided into four parts, agreement, personal information, checklist proper, and percentage. The data gathered was treated with descriptive analysis and cross tabbing. Results: The top 5 practices are: (1) humor, (2) giving clear and concise instructions, (3) engaging students in class, (4) asking questions that will make the students think, (5) giving examples that can be observed in real-life and examples that are relatable. Conclusions: The teachers and students alike call for an education curriculum that is timely, relevant, and engaging. A curriculum that will not only answer to the need of mind but also to the need of the body and the heart. To have a meaningful learning, the path of instructions must be set clear at the very beginning of every lesson to give a sense of direction for the students and for them to know what the end goal we are trying to achieve.

    Takotsubo Cardiomyopathy (aka Stress-induced cardiomyopathy) is characterized by a regional, usually transitory systolic disfunction of the left ventricle which is clinically difficult to differentiate from an acute myocardial infarction with no angiographic evidence of acute coronary obstruction. It occurs in approximately 1-2% of patients with probable myocardial infarction with troponin elevation. Electrocardiograms usually present dynamic changes compatible with myocardial infarction such as elevation of ST segment or may present Pointes Torsade or ventricular tachycardia. A 93-year-old woman admitted in the emergency room with a right hip fracture that requires surgical treatment. She is separated from her family members and taken to the operating room. She gets emotionally distraught on the way to the operating room due to separation anxiety and, upon arrival, refers chest pain and nausea. Immediately after, the patient becomes unresponsive without a pulse. Ventricular Tachycardia is documented, and the patient is defibrillated and there is a return to sinus rhythm. A 12-lead electrocardiogram shows ST elevation. Surgery is cancelled and an angiography is urgently performed with no evidence of coronary disease. Takotsubo Cardiomyopathy is diagnosed. The patient dies 7 days later. Takotsubo Cardiomyopathy, as well as its complications, is not always a reversible and transitory condition and may be deadly even with appropriate management and surveillance.

    We present the case of a 15 year old boy, athlete and American football player, who after receiving a contusion with the helmet of a player from the opposing team on his right thigh, began with sudden and disabling pain, however, despite of the initial anti-inflammatory treatment with physical measures and once the presence of femoral fracture and hematoma had been ruled out, it persisted with symptoms highly suggestive of compartment syndrome. Due to the high rates of morbidity and mortality that can occur in an untreated compartment syndrome, we decided on emergency surgical management, which consisted of a lateral and posterior fasciotomy of the right thigh and delayed primary closure 2 days later . The patient was discharged due to clinical improvement and returned to competitive sports activity 4 months after the surgical procedure.

    Background: Cataracts are among the main causes of blindness in middle- and low-income countries; however, their incidence has decreased in recent years due to an increase in cataract removable surgery (phacoemulsification). The anesthesia management for phacoemulsification has changed throughout the years; currently, most cataract removable surgeries are performed under topical anesthesia and intravenous sedation. Dexmedetomidine at high doses (0.5-1 mcg/kg) has been used for sedation for cataract removable surgery with a significant decrease in postoperative pain, low incidence of nausea and vomiting with a significant hypotension and bradycardia as well as a delay in hospital discharge. Low doses of dexmedetomidine (0.2 mcg/kg) have not been studied for cataract surgery (phacoemulsification). Objective: The objective of this study is to evaluate low dose dexmedetomidine (0.2 mcg/kg) to diminish postoperative pain in cataract removable surgery. Materials and Methods: 100 patients undergoing phacoemulsification under topical anesthesia and sedation, ages 40 to 92, ASA 1-II were included. Patients were randomized into two groups: group with dexmedetomidine and group without dexmedetomidine. Both groups received midazolam 20 mcg/kg and fentanyl 1 mcg/kg as well as topical tetracaine 0.5% on surgical site. The group with dexmedetomidine received a single dose of dexmedetomidine 0.2 mcg/kg over 10 minutes. Sociodemographic data was collected from both groups and vital signs were recorded. A trained nurse documented the incidence of postoperative pain in the recovery room until hospital discharge, as well as time spent in the recovery room. Results: Patients had an average age of de 68.8 ± 10.6 years. Ten patients in the dexmedetomidine group (20%) had postoperative pain compared with 2 patients (4%) without dexmedetomidine [OR 6.0; 1.2 - 29; p =0.028]. The incidence of hypotension was statistically significant in the group with dexmedetomidine [RR 1.5 (0.9-2.3); p<0.01]. The group with dexmedetomidine had a recovery time of 70.2 ± 50.9 minutes vs.51.6 ± 26.7 in the group without dexmedetomidine group (p=0.02). Conclusion: Low dose dexmedetomidine (0.2 mcg/kg) is associated with a lower incidence of postoperative pain in patients undergoing phacoemulsification and is therefore an effective adjuvant for this type of surgery.

    The COVID-19 pandemic, also known as the coronavirus pandemic, is an ongoing global pandemic of coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The outbreak hit the world with unprecedented consequences on global health, economy and people’s lives. Going to work during this pandemic has placed frontline workers under immense and unprecedented pressure, putting their physical, mental and social well-being at risk. Exposure to excessive stress, for prolonged periods can have many harmful consequences on the emotional and mental well-being of frontline workers.The objective of this paper is to explore the challenges faced by the frontline workers (doctors, nurses and community health workers) during COVID 19 in different countries across the world.The literature search analyzed only peer-reviewed papers associated with challenges faced by frontline workers during COVID 19 pandemic across the globe.This study reports on the psycho-social health and well-being of front line health workers during this pandemic. It is important that employers and organisations recognise the challenges this vital workforce face in times of pandemics and implement appropriate support for these workers.

    Fires in the operating room are relatively rare and may be catastrophic. Most are mild, however, some result in severe damage, disfiguration and even death. All surgeries are at risk of fire specially in high oxygen concentration areas. Fires in the operating room can be preventable with adequate attention to oxygen supply. Jett Plasma Medical is a DC low-power electrocauter used in dermatology and ophthalmology for aesthetic treatments. Unlike other similar products, Jett Plasma covers a surface area of 1 mm2 per beam. A 67-year-old woman scheduled for a blepharoplasty and Jett Plasma in periocular area under sedation. The patient is provided with supplementary oxygen at 3 L/min through nasal cannula. Sedation is initiated maintaining a Ramsay 3 and spontaneous ventilation with supplementary oxygen. ChloraPrep is used to clean the surgical areas before starting the blepharoplasty. After this procedure is done, the surgical drapes are removed. ChloraPrepis used to clean skin before using Jett Plasma. A spark of Jett Plasma starts a fire on the patient’s face which rapidly expands towards the nasal cannula, immediately burning them. The anesthesiologist quickly removes the burning nasal cannula while turning the oxygen off and the surgeon removes the Jett Plasma from the patient. A nasal and oropharyngeal exploration is performed which revealed burnt nostrils and philtrum, the rest is normal. In the recovery room the patient refers pain at inspiration and when swallowing; her vital signs remain normal and her oxygen saturation without supplementary oxygen is normal. The patient is referred to otorhinolaryngologist who diagnoses second degree burns in both nostrils and continues with outpatient treatment. The incidence of fire in the operating room associated to Jett Plasma had not been reported previously; however, it must be used cautiously considering it may potentially ignite a fire especially if applied on patients with supplementary oxygen.

    ABSTRACT:

    The objective of this paper is to assess the with respect to access to eye care services in The Gambia. More specifically in this paper, the scorecard assesses the level at which people have access to eye care services. The research employed both quantitative and qualitative methods. The findings disclosed that in the Gambia, majority of eye care service centers are in the urban areas which leaves the rural dwellers with no choice but to travel long distances to access eye care services facilities. According to the findings of the FGDs, greater majority of the population expressed concern about the inadequacy of health services, which adversely affected access. This is further frustrated by the poor road conditions in the country outside of the Greater Banjul Area, especially in the North Bank Regions of River Gambia and the high cost of transportation to access services. Poor road conditions, inadequate eye care facilities and personnel made access very difficult with its associated requirements in the rural areas. This has prompted many rural communities’ recourse to alternative sources of care including traditional healers and patent medicine sellers. The study revealed that over 80% of the respondents reported that the nearest eye care services are accessed through hospitals and clinics. This is a demonstration that access to eye care services are generally at hospitals and clinics but not specialized eye care centers. Majority of households (80%) have access to formal eye care services as opposed to only 19.8 not having access to formal eye care services. The findings of both the quantitative and FGD revealed that cost is considered another bottleneck to access to eye care service in the country especially to rural communities. Based on the findings, practical implications and an agenda for future research are suggested. For instance, the study recommends: improving the road networks, constructing more vision centers not only in urban and peri-urban center but also in rural communities. Additionally, facilities and specialist in eye care related services can be provided at health facilities across the country to provide the muchneeded eye care services to rural communities.

    Introduction: A large proportion of the public in some countries are known to patronize the private sector for their health needs, and some users even consider these services to be more attractive. Objective: The study aims to explore the experiences and issues associated with private health service delivery among private health practitioners in Nigeria. Methodology: A descriptive cross-sectional study was carried out among private health professional at two national events, using self-administered semi-structured questionnaires. Data obtained was formed into tables and analysed using the IBM Statistical Package for the Social Sciences (SPSS) version 20.0. Results: Eighty-one (48.8%) respondents used rented facility for their private healthcare practice. The net worth of private health facilities (all-inclusive in naira) was stated by 53 (31.9%) respondents to be less than 10 million (23,980.24 USD). While some new health facilities were being established, others were closing down for various reasons. There were some concerns expressed on multiple taxations,third-party mode of payment/health management organization (HMO), public power supply and security in area of private health businesses. It was the wish of 89 (53.6%) respondents that government should provide enabling environment for private health businesses to thrive. Conclusion: Small-and-medium-size businesses that used rented apartments with a relatively low net worth dominated the private healthcare industry. Some of such businesses have failed for several reasons. We advocate for healthcare reforms in Nigeria that should encourage growth in the private health sector.

    In the past few years, there have been significant initiatives to increase low - income people's access tofinancial services, first with microfinance and then moving on to micro-insurance. Insurance businessesincur inherent risks in serving the low-end market due to the complexity of insurance and the fragility ofthe target population. This study identifies these risks, explains the measures used by Nigerian insurance companies to mitigate them, and suggests innovative ways to reduce them. Twelvecompanies that offer micro-insurance products in Nigeria were selected using purposive sampling, andone hundred and twenty key informants responded to the survey. Percentages, means, and standarddeviations were calculated to aid in the description of the data. Pearson Correlations and Chi-SquareTests were performed to determine the level of correlation between the variables of interest. Lowpenetration, limited distribution channels, correlation risks, policy lapse, product design, pricing, adverseselection, and a rigid regulatory framework are all risks that micro-insurance providers face, according to the survey. Use of technology to reduce administration costs, control of moral hazard and adverseselection, rigorous investigation of claims, development of risk measuring models, public awarenesscampaigns, and constant monitoring of clients are some of the measures being employed to mitigatethe risks. To effectively reduce micro-insurance risks, micro-insurance service providers should invest inresearch and actuarial services to optimize product pricing, develop innovative distribution channels, build technology-conscious partnerships, establishRisk reference bureaus where clients' risk profiles can be shared and exploit new flexible premium payment terms. The National Insurance Commission (NAICOM), the industry regulator, is also urged to help ensure that micro-insurance policies are written in plain language that clients easily understand.

    In the course of our long-term practice in the clinical application of the principles of systematic approach and those of Integrative Oncology we purposefully and gradually began to develop all available achievements of integrative biophysics. In 2018, after a long study period, we created the IPT & BPT method, which is a combined application of Insulin Potentiation Therapy and Biomagnetic Therapy with Magnetic Pairs. The first successful results of the application of this method were presented in two consecutive publications (1,2). The development and achievements of quantum physics offered the opportunity to build the concept of biological energy and information transfer in the field of medicine. Our research in this field using scalar energy was reflected in our literature review (3) . Based on these scientific advances, we focused initially on streamlining the application of Biomagnetic Therapy with Magnetic Pairs using scalar information transfer to correct magnetic depolarization. Subsequently, we developed the protocol for scalar IPT & BPT replacing the intravenous administration of chemotherapeutics with scalar transfer of the same drugs. The treatment is initially aimed only at patients in the final stages of the disease who are subject to palliative care and those who refuse to use conventional methods. In the period from July 2020 to January 2022 the treatment was administered to 17 patients. Our first results showed that it proceeds without any side effects and with a significant improvement in the condition of patients. Three cases from our clinical practice on the application of Scalar IPT & BPT are presented demonstrating real possibilities for the application of the method for the treatment of oncological diseases. Initial clinical experience in the application of scalar energy and information transfer and the presented results demonstrate a new opportunity and perspective for the need to integrate methods of modern biophysics in the complex treatment of cancer.

    In the Audit Industry, there are a lot of players but there is major Four firms that are mainly taken into consideration which are EY, KPMG, PWC, and Deloitte and Touche. These firms are expected to have quality standards when it comes to ethical practice and understanding as they are regarded as well-trained professionals and third-party entities. However, there seems to be a contrary standpoint as these firms are found wanting in some of the corporate scandals in which the firms were fined huge sums of money. In this regard, the researcher found it worthwhile to find the causes of the audit scandals, whether there is an adequate ethics component in Chartered Accountants training and whether there is a link between audit scandals and Auditor social background. In this study, both qualitative and quantitative research approaches were employed while utilizing a descriptive case study research design. From a sample of 80 questionnaires, 40 responded and 40 did not respond and a 50% response rate was achieved. The population comprised of auditors from EY, KPMG, Deloitt,e and PWC. Questionnaires were used to collect data through and responses were based on the Likert scale. Data was analyzed and presented using the Statistical Package for the Social Science tool and Microsoft Excel. The findings of the study are that the major causes of audit firms are lack of professionalism and failure of honesty. Using descriptive statistics, it shows that there is adequacy of ethics component in chartered accountants training. Using regression analysis, it shows that the auditor’s interpersonal skills and auditor’s character contribute to the audit scandal. Meaning there is a link between scandals and social background. However, in order to change the narrative Audit firms should prioritize regular ethical training and have strict disciplinary actions for those who are found on the wanting side.
